Parent/Guardian Consent & Acknowledgment

Please read each statement carefully and tick every box. All are required.

1.I confirm that I am the parent/legal guardian of the participant being registered for the GIANTS Teens Camp taking place from 12th–15th August at Captain James Camp, Naivasha.

2.I give permission for my child to participate in all authorized camp activities, including mentorship sessions, recreational activities, swimming, transportation, team-building activities, and supervised camp programs.

3.I understand that while reasonable safety measures and supervision will be provided, participation in camp activities may involve certain risks, including accidents, illness, injury, or unforeseen circumstances. I authorize the camp organizers to seek emergency medical treatment for my child if necessary and if I cannot be reached immediately.

4.I confirm that I have disclosed all relevant medical information, allergies, medications, behavioral concerns, or special needs relating to my child.

5.I acknowledge that the GIANTS Teens Program, its organizers, volunteers, facilitators, venue hosts, and affiliated representatives shall not be held responsible for injuries, loss, theft, damage, illness, or incidents occurring during the camp except in cases of proven gross negligence or unlawful misconduct.

6.I also grant permission for photographs, videos, and media recordings taken during camp activities to be used for ministry, educational, promotional, and social media purposes by the GIANTS Teens Program.

7.I understand that all participants are expected to follow camp rules and maintain respectful conduct. Serious misconduct or violation of camp rules may result in dismissal from the camp without refund where applicable.

8.By checking this final confirmation box, I confirm that I have read, understood, and agreed to all the terms above.
